What to expect

Being well-prepared for your visit will ensure that the doctor has all of the necessary information required to render optimal care. Please plan to arrive 15 minutes prior to your appointment so that our caring staff may facilitate completion of any required forms, receipt of any required copays and so that our staff may prepare you for your visit with your doctor.

What to bring

• photo iddriver’s license, student ID, senior ID
• insurance cardif you will be paying with insurance
• copay paymentif your insurance requires copays, we require payment to be made at the time of the visit. We accept most major credit cards, personal checks and cash.
• pertinent medical recordsif you have had care provided by another dermatologist, please bring your medical records so we can review these with you, if needed.
• completed patient formsWe encourage all new patients to visit our patient portal. Alternatively, you may instead download and complete the New Patient Form and Medical History Forms found below. Please also review our Privacy Policy and Financial Policy.
